How to properly fill out a declaration when shipping from the USA to Moscow and Kyiv?

The most common form is sample 2976-A. The customs declaration looks like in the first or second photo, and may be in Russian or English.
It is important to fill out the document correctly, otherwise delays may arise — the item may be held for an indefinite period or not allowed through at all.
The information that must be provided to the seller (if you are using mail forwarders, for example Pochtoy.com — this information must be entered in your personal account):
- Full name;
- Passport details (series, number, by which authority and when it was issued);
- TIN (tax identification number);
- Phone number and email address — so that you can be contacted;
- A detailed description of all purchases. Separately indicate the prices for each item, and the total amount at the end.
Without a TIN and passport details, customs may not allow the parcel through and may send it back! However, in most cases customs officers request this data from you. This is visible in our account.

Why can a package be delayed at customs?
The most common reason for a delay is incorrect completion of the customs declaration. For example, you bought an item for $400, but indicated $350 in the document.
It is necessary to fully specify the names of the items. The fields must be filled out in English. The description must include: the type of item, its model, material, size, and trademark (brand).
